Same item twice in one review

I just complete a short review (10 questions) and presented with the institutionalization vocab twice despite marking it correctly in English and Japanese the first time. Is this normal?

This usually happens with me when I either add a synonym or use the reorder script.

you know I did add a synonym the first time, that must have been the cause. Thanks for the insight.

to second, always happens to me if I add a synonym. I wouldnt worry about it much!
