I think the とうきょう is supposed to only refer to the second kanji, to remind you of the unusual とう reading for 頭。
It says, ‘You haven’t learned this reading for 頭 yet, so here’s a mnemonic:’
They’re assuming you already remember the こう readings for 喉 and 叩。
