That’s a new one ^^. Do you have any scripts running at the moment? Stuff like Katakana Madness is known to cause issues.
If not I would recommend contacting WK staff.

I’m using Ultimate Timeline and Dashboard Progress Plus so it shouldn’t be it.

Did you have more than one tab open at the time? Multiple Wanikani tabs have also been known to cause problems during reviews
