I assume “Number of items guru’d in a Level” refers to the FIRST time you guru something? Like I have some items from earlier levels that went back to apprentice when I couldn’t recall them, but the Level progress charts did not change to reflect this

Yes, you are correct, SenseiLutzy. It only count’s the first time you guru’d the items. It does make sense, if we think about it as achievement (achieved guru?), not permanent state (still guru?).
For example my level 4 item 文字 (letter, character, text) dropped to apprentice, but my level 4 stats still show “You’ve passed this level, and you’ve Guru’d all items. Great job!”. It happened to me with kanji as well in the past.
And this makes sense. I think you’ll find that you can often re-learn the demoted kanji more easily than guru-ing them the first time around.
