Does it matter if I get reviews wrong more than once?

For example, suppose I have an item that is Enlightened.

When comes up in a review, I answer the pronunciation wrong, then I get the meaning wrong, and then when it asks me the pronunciation wrong, I get it wrong once more.

Does this count as just one failure, or upon repeated failures during the same session, could the item move lower and lower, ending up in Apprentice?

Yes, it drops lower for every wrong answer
