For the English readings I tried lowercase + uppercase and different synonyms but nothing worked.

Another user identified that this seems to be a problem caused by the Katakana Madness userscript. Until we get a fix for it, it might be best to disable that script during lessons.
Unless you don’t have this script installed, haha, in which case I do not know what could be causing this.
