I think this is an error? I see “Form” in the box and I’m not sure what to make of it (although, I am a bit rusty currently, I don’t remember if I’ve seen English words there before…)

Or I guess maybe it’s supposed to be “く Form”, and it has an extraneous 〜?

Look at the list shown, they are all of the form 辛く[something here]
