I imagine the thing that is wrong is the alternative meanings. It should be “name and address.” 手紙や書類などに書く、先方の氏名。また、 **住所と氏名** 。名宛。 ---

Or they told the intern “Add ‘name and address’ as a synonym for 宛名” and were misunderstood… 😅 ---
