When I do a group of 5 lessons, one item will come up for review in 2 hours and the other 4 items don’t come up for 4 hours. Why? Shouldn’t they all be the same?

Levels 1 and 2 are on an accelerated schedule to help you get started quickly, and level 3 is the first “normal speed” level. In all honesty, it would be nice (for many people, anyway) to have a few more accelerated levels… like maybe 3 and 4. But I think WK avoided doing that because they don’t want people accusing them of false advertising by slowing things down as soon as you subscribe after level 3. So you get to experience at least 1 normal-speed level before the free part ends.
