Sometimes, just as I commit to answering a question - i.e. as my thumb is going down, but immediately before pressing the button - I realize my answer is wrong, but too late! I have answered incorrectly.
Anyone else?

Oh this happens to me all the time. And then I curse myself. But I guess if I answered wrong on a glance I don’t know it well enough, so 
The physical unit of time between hitting enter and knowing you are wrong is called an ohnosecond.
It happens to me quite often, actually.
In many cases I’m too quick to press enter and end up answering wrong simply because I wasn’t paying enough attention and didn’t give myself a second to check what I’ve written… 
ALL THE TIME! I try to be very careful and double check if I got the meaning correct, spelled it correct, and pronounced it correct. Taking it one step at a time instead of just rushing helps me. Please let me know if this helped you or not.

That’s why I have an undo script installed.

But yes, it does happen a lot, even more now that I don’t have any pressure to be careful…
That’s why I have an undo script, but make it so that I can’t use it on burn items
